Hospice of Northen Colorado

Hospice is about choices. We are focused on giving patients and families a genuine voice in determining the end-of-life care that best suits their needs. Community education is a key in helping patients and families make informed decisions. Often Americans are reluctant to discuss end-of-life care with either their loved ones or their healthcare providers. Many Americans are not fully aware of the hospice Medicare benefit; many healthcare providers do not voluntarily offer such information at the earliest appropriate time.

Hospice of Northern Colorado is committed to educating the public in Weld, Morgan and Larimer counties about hospice care and end of life issues. We have a team of specialists available to make presentations to you, your workplace, social groups, clubs or organizations. HNC supports the use of 5 Wishes. This is an easy to use booklet that all people over the age of 18 should utilize. It spells out what you’d like the end of your life to look like. This booklet ensures your wishes will be carried out in the event you are unable to communicate your wishes to family and/or healthcare workers.

HNC is also available to respond to workplaces, schools or organizations following a death. Our organization employs trained bereavement workers, who have extensive experience in grief counseling.
