Entering Our Program
Hospice of Northern Colorado provides end of life care to people facing incurable illness. Such care differs from curative treatments, which focuses on restoring health and instead is supportive care for the person with a terminal chronic illness.
The goal of our care is to provide comfort for the rest of the patient’s life, and ending in a peaceful death. HNC’s focus is on relieving the symptoms of an incurable illness such as pain, shortness of breath, nausea and constipation. We address the psychological, social and spiritual needs of the patient and provide support for the patient’s family and caregivers by giving 13 months of free bereavement care after a patient’s death.
We provide care through our medical director, nurses, CNA’s, social workers, chaplains and volunteers. Our hospice care includes prescription drugs, over the counter medications, medical supplies and home medical equipment needed to address the terminal illness.
A phone call to Hospice of Northern Colorado at (970) 352-8487 or (800) 564-5563 can begin the admission process. Anyone can make a referral to Hospice of Northern Colorado. The earlier the referral the more beneficial it is to patients and their families. Final approval for our hospice program must come from the patient’s primary care physician.
Once approval is obtained, our hospice admissions nurse schedules a visit to explain the program and services and assesses the needs of each patient. Together, the patient and family make the choice to begin hospice care. Admissions are available to patients regardless of race, religion, national origin, sexual orientation, handicap, age, diagnosis or ability to pay.
