Memorial/Annual Giving
Donations are depended on for Hospice of Northern Colorado to continue our mission of providing quality end-of-life care to individuals living with a terminal illness. If you are considering giving to Hospice of Northern Colorado, we would like to thank you in advance. Your gift is critical to our organization so we may continue to provide care to those in need of hospice services in Weld, Larimer and Morgan Counties. All gifts are tax deductable. A receipt will be issued to you upon receipt of your gift.
The following are ways you can give to Hospice of Northern Colorado:
Memorial or Tribute Gifts
Memorial gifts in memory of a loved one, or tribute gifts in honor of a loved one, is one way we raise needed funds to continue our mission. The family of the person memorialized or honored will be notified of your generous gift but will no amount will be named.
Gifts can be given on our website. Click her to be directed to our online donation form. You may drop off cash or checks Monday-Friday from 8 am – 4:30 pm at 2726 W. 11th Street Road, Greeley. You may also mail your gift to:

Please acknowledge who your gift honors either on the memo of your check or on a separate sheet of paper to enclose with your gift. If the gift is given in memory of an individual who was not in our services, please include the name and address of the family so we can send them an acknowledgment.
Annual Gifts
We accept general donations any time. These gifts do not need to be in memory or tribute to anyone and will be used to take care of patients in our community! We can accept these gifts in the following ways:

Monthly Gift
A convenient way to make a donation to Hospice of Northern Colorado is through a monthly gift. Your gift can be withdrawn from your bank account or charged to your credit card at the same time each month. If you are interested please contact Amy Dobersen, Community Development and Fundraising Coordinator at (970) 352-8487 extension 147 or at adobersen@hpcnc.org. You may opt out of this program at any time.
